If so, please fill out the attached form and send it to your employer as listed in the form.
- If you received pay statements through Canada Post or Sogica systems, send them to the SAP email address.
- If through your employer, find your specific employer’s email on the list.
Deadline: April 16, 2025.
All former members of Local 204 – under the WRHA and Shared Health umbrellas have until April 16, 2025 to send in this form to their prior employer.
